A Mental Health and Substance Use Services Collaborative Framework for Patient-Centred Outpatient Care

  • Please describe how you meet the criteria, which can include personal, professional and volunteer experience (including roles on committees/boards) as it relates to this committee. (Though we ask you as a patient partner to 'leave your hat at the door,' it is useful to know your professional background and experience.)
  • The BC Patient Safety & Quality Council will collect personal information via Word Press under section 26 (c) of the Freedom of Information and Protection of Privacy Act for the purposes of operating the Patient Voices Network. Personal Information submitted by volunteers will be saved and stored on file by BCPSQC for 5-10 years upon submission.
