The Virtual Care Strategy branch of the Ministry of Health is developing a provincial virtual home care strategy and we need the patient voice! We are looking to engage and consult patient partners through review of three online surveys to ensure our survey centres the patient.

Aim

Our Home Health Monitoring (HHM) projects engage clients and their families to support patients’ quality of life by educating patients and caregivers about chronic conditions and symptoms, and giving patients the freedom to self-manage their condition(s) from their homes and away from traditional hospital settings. We recently created five surveys: three for patients with chronic disease (e.g. heart disease, COPD, multi-morbidity), one for caregivers and one for care providers to assess their experience and satisfaction with the HHM program during and after completion. We would like to involve patient partners to review our surveys and give us their feedback on the appropriateness of the survey questions.

Level of Engagement

This opportunity is at the level of involve on the spectrum of engagement. The promise to you is that the health care partner will involve patients in planning and design phases to ensure ideas or concerns are considered and reflected in alternatives and recommendations.

Eligibility

  • Has experience with health monitoring at home;
  • Is familiar with chronic heart disease and COPD;
  • Must have access to the internet and a computer

Logistics

Vacancies: 3 Time, Date, Location: The Ministry of Health will email the five draft surveys for review and a survey link to gather patient partner feedback on the patient's own time. Commitment: A phone call with the health care partner to review the background information and the surveys. The review of the five surveys is estimated to take 60-90 minutes. A possible follow up call to elaborate or clarify any feedback from the patient partner may also be requested.

No expenses anticipated.
