The Providence Health Care Heart Centre is looking to improve a patient’s access to support and resources to quit smoking (smoking cessation) when hospitalized and to create a clearer path for follow up after discharge. Smoking can damage your heart health so it is important to help patients quit smoking when they have been hospitalized with a heart problem.
This knowledge translation project will improve the patient experience by providing access to smoking cessation experts (team that has specialized knowledge of quitting smoking and counseling). These smoking cessation experts can provide direction and help to the team caring for the patient trying to quit smoking while hospitalized. The smoking cessation experts will follow the patient through their hospital stay and making a plan for the patient to get follow up within their community. This project will also include identifying and approaching smoking or recently quit smoking patients sooner when hospitalized.
Our knowledge translation project team is looking for a patient partner to participate in this project by sharing their lived experience and insights to help shape our project plan.
Patient/Family Partner Involvement:
• Patient partner will act in a consulting role to offer their insights and suggestions for how to implement the smoking cessation project and pathway for patients.
• Patient partner will provide feedback on materials that have been curated to support smoking cessation.
Time Commitment:
• 1 hour virtual (Zoom) or in-person meetings approximately every 2 months
• 6 month time commitment starting in January/February 2024
• Additional email communication outside of meeting times may be required
Honoraria will be provided.

- Willing & comfortable to share health care experiences in a group setting
- • Experience as a patient who was a smoker while they were admitted to hospital - Ideally experience with inpatient admission on Cardiac unit or history of heart problem
