Aim
Queens Park Care Centre have two quality improvement projects that aim to improve the patient experience and improve that quality of care. Work with the teams to ensure goal planning and family meetings meet the needs of patients and families.
Goal Planning: The neurology rehabilitation unit are shifting the way they develop weekly goals. They would like to make sure patients are involved in goal setting and developing a communication tool to make sure everyone is aware of the plan.
Family Meetings: The Musculoskeletal Rehabilitation Unit (MSK) is looking to improve family meetings to make sure they are person centred and strength based.

This opportunity is at the level of consult on the spectrum of engagement. The promise to you is that the health care partner will listen to and acknowledge your ideas and concerns, and provide feedback on how your input affected the decision.
Eligibility
Open to volunteers from across Fraser Health who:
• Experience with rehabilitation services
• Have access to a telephone or computer for videoconferencing
Logistics
• Number of vacancies: 4
• Date/Time: Meet with the teams (in person or virtually) at a time that works best for you.
• Commitment: Up to six months.
• Location: In person or virtual.
Reimbursement
If required, travel expenses will be reimbursed.
Background
Fraser Health values and promotes continuous quality improvement. Queens Park Care Centre is identifying areas that could be more person centred to improve the care experience. Small teams are working together to improve the quality of care.
Queen’s Park Care Centre offers inpatient rehabilitation services to adults (19 and over) living in the Fraser Health region. The three in-patient units are:
– Stroke Rehabilitation Unit
– Neuro Rehabilitation Unit
– Musculoskeletal Rehabilitation Unit (MSK)
Rehabilitation includes a variety of therapies provided by a variety of healthcare providers. The rehabilitation can include physicians, nurses, physiotherapists, occupational therapists, rehabilitation assistants, speech language pathologists, social workers, dieticians, and pharmacists.
