Optimizing chronic disease management is one of the Fraser Health Primary Health Care area of focus. The goal is to support the people we serve to manage chronic health conditions in the community and reduce hospital admissions. To reach this goal it is important for Fraser Health to gain an understanding about the perspective of the people we serve and learn more about individual healthcare journeys.
To begin with, the team plans to map out the services and identify the gaps for two common chronic health conditions, Chronic Obstructive Pulmonary Disease (COPD) and Chronic Heart Failure (CHF). Meet 1:1 with the team to share what is important to you and share your ideas to make sure the community services meet the needs of the people we serve. You will also be invited to participate in one group session at Central City in Surrey.
