In order to shape primary health care in Ridge Meadows, a variety of experiences and voices are needed at the table.  The Ridge Meadows Primary Care Network brings together community stakeholders to create partnerships and identify opportunities to improve how community members can access good quality health care in their community, across all ages and needs.  They’re looking for two patient partners to join them in their efforts by providing feedback on Primary Care Network services.

Aim

  • The team will focus on the specific needs of the community and improve health services identified as high priority for the community, including moms and babies; maternity care; Indigenous health; an integrated chronic-care program: adult, child and youth mental health; chronic pain; substance use; and seniors care and prevention.
  • The patient partner will help identify partnership opportunities and give us input and evaluation wisdom on our patient care improvement strategies.

Level of Engagement

This opportunity is at the level of consult on the spectrum of engagement. The promise to you is that the health care partner will listen to and acknowledge your ideas and concerns, and provide feedback on how your input affected the decision.

Eligibility

If you have a strong interest in this work but have not yet completed a PVN orientation and volunteer agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact the engagement leader directly.

Logistics

  • Date/time: Two meetings a year, to be determined with patient partners
  • Location: Ridge Meadows
  • Commitment: One year, with an opportunity to extend

Reimbursement

Preapproved expenses, per Division of Family Practice Travel Policy, related to local travel to in person meetings will be reimbursed including mileage, parking and/or public transit.
