This document needs to contain enough patient information to be user friendly and serve as a clear reminder of what the next steps are to book in for chronic disease management appointments if you do not have a primary care provider. We have quite a spectrum of how much information we can provide but we don’t want patients to be overwhelmed or they will not use the document. Patient feedback is imperative to our success in this health care collaborative venture

  • Willing & comfortable to share health care experiences in a group setting
  • Specific health care experiences [details below]
  • Previous committee/working group experience
  • Comfortable using technology to attend online/virtual meetings
  • An interest in improving health care services
  • interest in creating sustainability in primary care. especially for those that do not have a primary care physician
Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
