Aim

To provide patient input into the implementation process, tool development and monitoring of  support for health care teams and residents and families.

Level of Engagement

This opportunity is at the levels of consult and involve on the spectrum of engagement. The promise to you is that the health care partner will listen to and acknowledge your ideas and concerns, and provide feedback on how your input affected the decision; and involve patients in planning and design phases to ensure ideas or concerns are considered and reflected in alternatives and recommendations.

Logistics

• Vacancies: 2
• Date/time: the 4^th Tuesday of the month, from 11:00 am – 12:30 pm
• Commitment: 1-2 hours, once a month; one-year term. Potential for a short-term working group, on top of this commitment it would be explored with patient partners
• Location: Combination of in-person (Central City, Surrey), teleconference and webinar.

Reimbursement

Preapproved expenses related to travel to in person meetings will be reimbursed according to the Fraser Health Travel Policy (including mileage, parking, tolls and/or public transit).

Background

Over the past few years, Fraser Health Residential Care (RC) has been working on a broad plan to integrate a palliative approach to care across Fraser Health. In the early stages of development, they conducted focus group interviews with care teams and representatives of Patent Voices Network to integrate patients’, family and care team voices. Full implementation phase has begun, based on the pilot findings at two of the RC sites. These efforts include a quality improvement collaborative educating teams about a palliative approach and how to have serious illness conversations related to advance care planning. This collaborative approach is over the next 2 years. They are seeking two patient partners to be a part of the advisory committee responsible for informing and advising on implementation and monitoring of this approach, ensuring it is patient- and family-centred.
