Providence Health Care (PHC) is looking for patient/family partners to join the Scotiabank Youth Transition Program (SYTP) Working Group. Patient/family partners will collaborate in developing the SYTP and associated resources to support youth transitioning from pediatric care to multiple clinical areas at St. Paul’s Hospital.

In Fall 2023, a working group was established to include representatives from programs across PHC and BC Children’s Hospital (BCCH) to develop an overall vision for the SYTP and to begin work on specific resources needed to fulfill that vision. The working group has engaged with the Youth Advisory Committee (YAC) at BCCH to obtain feedback and validation of the work to date.

Patient partners will:
– Provide input in planning, design and implementation of the SYTP and associated resources.
– Use their experience to inform development of the SYTP so that it is relevant, meaningful and centred on youth patients, families and caregivers.

• Vacancies: 3 (1 family partner and 2 young adults, or 3 young adults)
• First meeting will be in January 2024.
• Meetings are typically 1.5 hours in length and there is often some additional time needed to review materials, agenda’s and meeting minutes.
• Correspondence will occur via email.
• Meetings occur M-F during business hours (9:00AM – 5:00PM).
• Meetings occur over Zoom.
• Meetings occur monthly.
• The length of the project is projected until 2026.
• Honoraria will be provided. Expenses related to travel (i.e. public transit, parking, approved mileage) to attend any in-person meetings at St. Paul’s Hospital will be reimbursed.

Level of Engagement

This opportunity is at the level of Involve on the IAP2 Spectrum of Public Participation.

Eligibility

  • We are looking for young adults, ideally those who are in transition or are currently transitioning to adult care (18-28)
  • Someone who has some lived experience with a health condition and experience with health services
  • Alternatively, this could be a parent or caregiver of a young adult who has transitioned recently (in past 3-5 years) or is currently transitioning to adult care
  • Someone who is comfortable using Zoom
  • Someone who is comfortable sharing their experiences with health care and transition
Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
