There are times in health care when something occurs (referred to as a Critical Patient Safety Incident) that results in severe harm or death. When a critical patient safety incident happens a quality review is done to develop recommendations to improve the quality and safety of care.

The quality review team is looking to include the a patient partner as a member of the quality review incident analysis team to bring the patient’s perspective to motivate health care teams to consider alternative perspectives and contribute to a deeper analysis or understanding of the underlying system contributing factors. The patient’s perspective will also be useful in providing input into practical solutions and patient-centred recommendations that aim to prevent an incident from occurring again.

Join the quality review team for the next year to participate in the Quality Review meetings. These meetings will be planned based on everyone’s availability and will include time to prepare before the review and debrief after the review.

Quality Reviews are a detailed system level analysis of critical patient safety incidents within a non-blaming atmosphere to identify the root causes and develop recommendations to improve the quality and safety of care. Quality Reviews are protected under Section 51 of the BC Evidence Act, which means that documents and discussions arising from these reviews cannot be disclosed outside of the review process or used in any legal proceedings.

Quality Review team members often include:
– Senior leaders
– Clinical operations staff (such as nurses, educators, managers, physicians, allied health)
– Quality Review Leader
– Quality Review Coordinator

This team is seeking a Patient Advisor/ Partner to:
Participate in Quality Reviews as part of the incident analysis team to:
• Identify pieces of the process that are confusing or missing from a patient and family perspective
• Discuss and analyze findings
• Assist in developing recommendations to mitigate the risk of recurrence and improve the quality and safety of care provided

Level of Engagement

This opportunity is at the level of Involve on the IAP2 Spectrum of Public Participation.

Eligibility

  • Willing & comfortable to share health care experiences in a group setting
  • Previous committee/working group experience
  • Have access to technology and the internet to participate in the engagement opportunity
  • An interest in improving health care services
  • The time to participate in the engagement opportunity
Patient Partners ARE required to be PVN members to participate in this engagement opportunity.
