Nanaimo Wound Care Project: Phase 5, Patient Self-Management Education Resources

Aim

• To create a consistent framework of patient wound care education and resources to be used by Physicians, Specialists, Nanaimo Regional General Hospital staff and Community Nurses.
• Identify gaps in current Wound Care resources and services.
• Create educational materials to fill those gaps.

Level of Engagement

This opportunity is at the level of collaborate (at a local community or program level) on the spectrum of engagement (www.iap2.org). Eligibility

• History of Wound Care needs in an acute setting.
• History of self-managing wounds as a patient or caregiver.
• Comfortable relaying their experiences.
• Ability to attend up to 4 evening meetings.

Please note: Patients should have previously attended a PVN orientation session. If you have not attended an orientation but are interested please contact the Engagement Leader directly to see if accommodations may be possible.

Logistics

• Vacancies: 2
• The Task Group will meet up to 4 times between December 2016 and April 2017.
• The first 3 evening meetings will occur approximately every 2 weeks (1 in December, then a holiday break, reconvening in January 2017). Meetings will be held from 6:00 PM to 8:00 PM at the Nanaimo Division of Family Practice office, #17 - 1601 Bowen Road, Nanaimo. Exact date of first meeting to be determined.
• In person attendance is preferred. Teleconference is possible if unable to attend all meetings.
• The education planning portion should be complete by the end of January, 2017. A public wound care forum will be held in April, 2017.

Reimbursement

Mileage reimbursed at .50 per km. Dinner will be provided at the meetings.

Background

The project's goal is to ensure patients receive the right care at the right time by improving overall processes and systems such as patient triage, referrals, patient progress notes and consult follow-up between referring and accepting physicians, and the sharing of consistent patient care pathways. This can be achieved through better coordination and integration of existing wound care services and working through the identified gaps in service availability and access and improving patient/provider and physician with physician relations and communications. The initiative encompasses all five tiers in the continuum of care identified in the Island Health Tiered Primary Care Model. The Patient Education portion of this initiative will involve Physicians, Specialists, Medical Office Assistants, Patient Partners, and Community Nurses. Self-Management focuses on wounds treated outside of the acute setting.
