Fraser Health is looking for people interested in senior’s health care services who can contribute their voice to the development of the Regional Older Adult Strategy and Framework. The Fraser Health Frail Elderly Older Adult Network seeks two patient partners to join their team in co-creating this strategy that will guide health care services across the region for seniors.

- To develop a Regional Older Adult Strategy and Framework to guide our health care services throughout the continuum of care
- The patient advisor will partner with the team during the development phase of the Strategy, sharing their insight on the successes, barriers, and gaps in service across the region

- Commitment: 2 meetings, over a 2-3-month time frame
Reimbursement
Preapproved expenses related to travel to in person meetings will be reimbursed according to the Fraser Health Travel policy (including mileage, parking, tolls and/or public transit).Background
The Frail Elderly-Older Adult Network Team was established in 2015 with a regional mandate in areas of physician recruitment, clinical quality, and service standardization for the older adult population. Overall, the goals of this team include:- Develop a Regional Older Adult framework;
- Re-develop an education and resource hub;
- Develop a new model of older adult care at Surrey Memorial Hospital;
- Include delirium as a patient safety priority;
- Optimize primary and community care redesign to support the medically complex frail elderly specialized population and standardize older adult care.
