Specialized Community Services Programs (SCSP) is a Ministry of Health directive and regional priority to support community-based primary care networks by providing well-integrated and coordinated specialized services for individuals with moderate to severe mental health and substance use (MHSU) conditions. The SCSP initiative would like to recruit two patient partners to sit on the steering committee and represent the patient/family voice. If you have experience with MHSU services within Vancouver Coastal Health and would like to support more integrated, coordinated, person- and family-centred specialist services please consider applying for this committee.

Key features include a single point of access for care, urgent response and triage services, most responsible clinician that is known to the family and primary care provider, and interdisciplinary team based model of care. Three SCSPs for MHSU will be established in the Vancouver Coastal Health region by March. There is regional oversight and coordination to ensure alignment between the SCSPs while respecting and fostering site specific initiatives.
