Emerging evidence demonstrates that co-designing improvements with patient and family partners leads to new insights and better outcomes than providers and leaders working on their own. But successfully implementing patient- and family- centred practices requires a commitment to supporting a culture shift to focus on what matters most to patients, families, and caregivers. Engaging patients successfully and building true partnerships starts with a willingness to embrace change.

Despite this challenge, health care organizations are using innovative patient- and family‑centred practices more and more, to successfully engage patients and families, improve health outcomes, and support ongoing quality improvement.

This interactive On Call session will explore leading patient- and family-centred practices and delve into 3 of these interrelated practices – family presence and participation in care (open visitation); bedside transfer of accountability; and the use of whiteboards – through first-hand experience from one organization, Huron Perth Healthcare Alliance.
