Fraser Health offers a wide range of home and community services that aim to help the people we serve to remain at home for as long as possible. Examples of community services that Fraser Health offers includes:
– Advanced Care Planning
– Caregiver Support
– Day programs for older adults
– Home Care nursing
– Home Support
– Nutrition and Dietician
– Palliative Care
– Rehabilitation Therapy
– Respite
– Seniors and Complex Care

Community Health Services Network Committee
A new committee has formed to guide, monitor, and collaborate with internal and external key partners related to Fraser Health community health services. This committee will develop and share a seniors, community, and complex care strategy and evaluation plan. The committee will also approve decision support tools (such as policies and guidelines) that aim to support a consistent, high quality approach across the region.

The committee values all perspectives and is looking to include the perspective of the people who have experience receiving community health services.

Participate in 4 virtual meetings a year to share your perspective. Next meeting is in September.

This opportunity is at the level of Collaborate on the IAP2 Spectrum of Public Participation.

Eligibility

  • Willing & comfortable to share health care experiences in a group setting
  • Have access to technology and the internet to participate in the engagement opportunity
  • An interest in improving health care services
  • The time to participate in the engagement opportunity

Patient Partners ARE required to be PVN members to participate in this engagement opportunity.
