Prevention Activity Survey – Rural

Study Purpose
Preventive care is a necessity for all Canadians, but among rural populations completion rates are particularly low. We would like to learn more about your experiences regarding your preventative health care practices, such as cancer screening, immunizations, and chronic disease prevention. We plan to use this information to increase education and access to preventative care while also empowering patients to take charge of their own care.

Details
Participants will be asked to complete a quick online survey on primary care provider attachment factors and preventive care service usage, completion, history, self-efficacy and preferences.

Survey participants will have the opportunity to enter a draw for a chance to win a $50 gift certificate.

Eligibility
Any individual over the age of 19, living in a rural BC community (population less than 20,000 people) is eligible to complete the survey.
