We are inviting patient partners volunteers to join a working/advisory group for a Medical Staff Model of Care Project. This initiative aims to refresh the current medical staff model of care (i.e., how health services are delivered by medical staff) to ensure the continued delivery of exceptional patient care, while also supporting PHC’s academic mission and the health and wellbeing of medical staff.

The project’s goal is to ensure that people receive the right care, at the right time, by the right team, and in the right place. Patient partners will help us explore the impact of provider-level changes and ensure that the patient experience remain central to our work.

As a member of the working group, the patient partner(s) will:
– Observe and advise on discussions and decisions
– Share lived experience to help shape a patient- and family-centred approach
– Help identify potential impacts on patients and families, including unintended consequences

This opportunity is at the level of Involve on the IAP2 Spectrum of Public Participation.

Eligibility

  • Willing & comfortable to share health care experiences in a group setting
  • Specific health care experiences [details below]
  • Have access to technology and the internet to participate in the engagement opportunity
  • Comfortable using technology to attend online/virtual meetings
  • An interest in improving health care services
  • The time to participate in the engagement opportunity
  • We welcome expressions of interest from individuals who meet some or all of the following criteria: - Have experience as an inpatient - Have navigated transitions in care (e.g., inpatient to outpatient, between teams or units, weekend to weekday) - Bring diverse lived experience and/or thoughtfully advocate for people with diverse identities, perspectives, and experiences
Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
