Have you had experience with accessing community care at home when you would otherwise require admission to hospital or extended stay in the Cariboo and Chilcotin? We are looking for patient partners to join our monthly steering committee for the development and implementation of our community programs that support clients with more complex care needs.

The purpose of engaging the patient partner is to make sure that we are being person and family centred care focussed as a committee. We will have an opportunity for patients to give their opinion at the committee and connect them with the Project Manager. The goal of the Steering Committee is to provide continuity, flexibility and rapid mobilization of responsive services to meet the needs of clients with higher community care needs.Level of Engagement
This opportunity is at the level of collaborate on the spectrum of engagement. The promise to you is that the health care partner will work together with you to formulate solutions and incorporate your advice and recommendations into the decisions to the maximum extent possible.Eligibility
• Experience as a patient, or support person, with community health services in the Cariboo and Chilcotin. • Preferred that you or your family have received care from community home care/support, allied health services or mental health and substance use services. • Comfortable either sharing their experience one on one or in a group setting. • If you have a strong interest in this work but have not yet completed a PVN orientation and Volunteer Agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact Cathy AlmostLogistics
• Vacancies: 2 • Date and Time: One hour Virtual meetings monthly from September 2020 to March 2021. Plus 1 hour prep and 1 hour with Project Manager, approx. 3 hours per month. • Method: Webex video conference platform • Commitment: seven months to a yearReimbursement
There are not any expenses anticipated for this meeting as it will be held virtually. If BC Health guidelines change, Interior Health will cover pre-approved travel and meal expenses through receipt reimbursement as per Interior Health policy.Background
This is a Ministry of Health initiative that includes Primary Care Network, First Nations Health Authority, and Interior Health community services and focussed on community care. The steering committee includes 30 stakeholders from all of the listed partners above plus 2 patient partners. The committee has been meeting for 2 years and could provide meeting minutes to the last meeting. Attendance by patient partners would be appreciated at our steering committee meetings, but we can also arrange private discussions with the project manager in addition and if preferred.Engagement Leader Contact Information
