Our project focuses on improving how disadvantaged or unhoused patients move between the hospital, community services, and social supports, especially those who face barriers such as unstable housing, complex health needs, or difficulty accessing care. We are improving ways for patients to receive better in hospital care & follow‑up care in the community, strengthening communication between providers and creating clearer pathways so people don’t fall through the cracks.

Having patient partners involved is essential because lived experience helps us understand what truly works, what feels supportive, and where the system creates unnecessary stress or delays. Patient input will shape the design of new care models, help us identify gaps we may not see from within the system and ensure the solutions reflect real needs rather than assumptions.

This opportunity includes participating in meetings, reviewing materials, and sharing perspectives on hospital‑to‑community transitions. Engagement can be flexible – virtual or in‑person. Your voice will directly influence how we design and test improvements that aim to make care more coordinated and culturally safe.

We are in the expression of interest phase with this project, with a proposal submission due by October 1st. Once approved, this project will move forward into the official project implementation phase with a guesstimate timeline of 1-1.5 years.
