University Health Network (UHN) in Toronto is conducting a Quality Improvement Project on Responsible AI: Patient Charter of Rights funded by the Future Skills Centre.

This project brings together patients, caregivers, health care professionals, and experts to ensure the health system understands what patients and caregivers expect and trust when Artificial Intelligence (AI) is used in health care.

• The survey will take about 15 – 20 minutes to complete. You may skip any question or stop at any time.
• You can enter a lucky draw to win a gift card.
