Aim
To enhance patient and family experience by reducing avoidable ER transfers from long-term care (LTC) facilities through comprehensive care protocols, improved communication and enhanced support systems.

Background
ER transfers from LTC facilities can be stressful and disruptive, often due to insufficient on-site medical care, inadequate staff training and poor communication. Many transfers are avoidable, causing unnecessary stress for residents and families and burdening the health care system. Families often feel frustrated due to inadequate communication and lack of involvement in decisions. Reducing unnecessary transfers can improve residents’ quality of life, family satisfaction and health care resource use.

Implementing strategies can reduce avoidable ER transfers, improving care consistency and overall experience for residents and families.
