Northern Health is working on a pilot project to improve how we communicate with families about school‑based immunizations. The goal is to create personalized immunization consent forms that include all immunizations a student is eligible for, not only the routinely offered Grade 6 or Grade 9 vaccines.
By including all eligible immunizations on one consent form, nurses may be able to offer and administer these vaccines during school clinics. This can help reduce barriers for parents and students who may otherwise face challenges accessing immunizations outside of the school setting.
Hearing directly from parents is an important part of this work. Parent input will help ensure the consent form is clear, respectful, and supports informed decision‑making.
We are seeking 1-5 parents or caregivers of a child in Grade 6 to Grade 12 in NH to review a draft personalized immunization consent form and suggest changes to improve clarity and usefulness for families.
Participation would involve reviewing the draft and providing feedback by email or during a short conversation. Feedback will directly inform how Northern Health improves access to immunization services.
