Aim
To improve RPM services through patient-led feedback.

Background
In Remote Patient Monitoring (RPM) patients with various chronic or episodic conditions complete health-directed questionnaires for their care teams using measurement devices (ex. Blood pressure monitor) in kits shipped to their home. The questionnaires are intended for use by their care teams to inform their care program.
Note: During this engagement, patients’ care teams will NOT be reviewing questionnaires submitted by patients.

Logistics
Patient partners feedback will be used to improve:
– Experience in obtaining remote monitoring kits
– Enhance patient experience for usability and perceived value

The engagement will consist of:
– An onboarding session (1h scheduled call)
– Using the solution ~15 mins, 3-5 times per week
– Completing 2 surveys (~30 mins each)
– An offboarding session (1h scheduled call)

Ideally, Patient Partner participants will include underserved populations – especially those in rural/remote settings and Indigenous communities – from every health region in the Province.

Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
