Delta Health Services is looking for two patient partners who have a passion for improving health care services in their home community. Patient partners have the opportunity to take on as many activities as they like and work as a team to support each other.

Aim

Patient partners bring their passion, creativity and experience to support health care services being more person- and family-centred through sharing your thoughts on how to deliver services to people like you. Patient partners will work alongside the health care team to create quality improvement strategies that are responsive to the needs of the community and creates an ongoing opportunity for input.

Level of Engagement

This opportunity is at the level of collaborate on the spectrum of engagement. The promise to you is that the health care partner will work together with you to formulate solutions and incorporate your advice and recommendations into the decisions to the maximum extent possible.

Eligibility

  • Is able to speak to a lived experience;
  • Excited to share their story in front of healthcare providers and leaders;
  • Experienced working in teams; and
  • Able to use email, online calendars and access Word documents
Patients should have previously attended a PVN orientation session.  If you have not attended an orientation but are interested please contact Jami Brown directly to see if accommodations may be possible.

Logistics

Vacancies: Two Time, Date, Location: To be determined by the patient partners. Meetings will take place at Delta Hospital. Commitment: One-year term, with the possibility of renewal. Flexible, based on your schedule. Most meetings happen Monday-Friday. Discreet opportunities are posted via email through the patient partner team. Please note: The current Patient Partner team will participate fully in your orientation and mentoring to begin this very interesting work.

Reimbursement

Preapproved expenses related to travel to in person meetings will be reimbursed according to the Fraser Health Travel policy (including mileage, parking and public transit).

Background

Delta Hospital has three currently active patient partners and is currently seeking more members to be a part of the numerous opportunities coming from acute care, residential and home care operations. You will be part of the Delta Patient Partner team and meet up with them at Peace Arch Hospital from time to time as initiatives are worked on cooperatively across the sites. There is an established Patient Partner Steering Committee that brings all concerns to the Executive Director monthly. The entire team of Patient Partners meets quarterly with the Executive Director and Site Directors to review all the activities that include patient partners and assess the outcomes of this type of partnership.
