The Chart Club at Providence Health Care (PHC) is looking for a patient and family partner to join their group. This is an opportunity to collaborate with health care professionals and help shape the quality and safety of care across the organization.

Chart Club is a methodology of reviewing patient charts to understand the system issues impacting care delivery. Every case is reviewed by two clinician reviewers independent of one another before the monthly meetings and then brought forward to chart club for discussion. Members of chart club participate in the discussion to collaboratively identify opportunities for improvement (OFIs).

Role of the Patient Partner
As a Patient Partner, you will be a member of the Chart Club, contributing your lived experience and insights during discussion. Your input will be used to inform decisions regarding opportunities for improvement from the patient/family perspective.

Time Commitment
• 1 hour 30 minutes virtual (Teams) meeting once per month
• No meetings in July, August and December

This opportunity is at the level of Involve on the IAP2 Spectrum of Public Participation.

Eligibility

  • Willing & comfortable to share health care experiences in a group setting
  • Specific health care experiences [details below]
  • Comfortable using technology to attend online/virtual meetings
  • - Has lived experience as a patient or family member at PHC - Wants to collaborate with a diverse group of PHC staff to help improve the healthcare system
Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
