If given the option, many individuals and families may prefer to receive care in the home instead of in a hospital. Be part of making this option a reality by planning a pilot program to deliver hospital level services in the home!

Aim

• Northern Health is developing a Hospital in the Home program to be prototyped in Prince George, and Terrace or Quesnel. This program will support the delivery of acute services in home with physician, nurse, diagnostic support as necessary Engaging patient partners early in the planning process will raise awareness of potential opportunities and barriers that should be considered in order to improve individual, family, and health care provider experience, lower health care costs, and enhance health outcomes

Level of Engagement

This opportunity is at the level of involve on the spectrum of engagement. The promise to you is that the health care partner will involve patients in planning and design phases to ensure ideas or concerns are considered and reflected in alternatives and recommendations.

Eligibility

• Open to patient partners from the Northern Region (ideally one from Prince George and another from a smaller, more rural community) • We are seeking someone with experience as a patient as well as someone with caregiving experience. Patient experience - older adult - has been hospitalized for chronic conditions - has been or currently is the recipient of home care services Caregiver experience - Experience caring for older adult (spouse, parent, family member) that fits the above criteria is desire • If you have a strong interest in this work but have not yet completed a PVN orientation and Volunteer Agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact Cathy Almost

Logistics

• Number of vacancies: 2 • Date and Time: 2 hours per week, Meet weekly or bi-weekly through the summer - until October 2020. Possibility of 1 or 2 half day meetings • Location: Meetings will be held via teleconference and videoconference • Commitment: one week to 6 months

Reimbursement

There are not any expenses anticipated for this meeting.

Background

The Hospital at Home program is being developed to understand how to provide appropriate level of care for individuals and their families in the most desirable setting. A successful Hospital at Home program would help patients and their caregivers to stay out of the hospital, and remain comfortably in their homes, even as their care needs may become more complex.
