BackgroundThe Division of Family Practice is putting together a proposal to improve coordination of care for patients and their caregivers, including transitions between hospital and community-based care, and communication between family physicians and specialists involved in you care. This opportunity is to identify current gaps in care for patients with congestive heart failure who have been discharged from the hospital. Input from patient partners will be used to help us identify current gaps in care, and areas where the system of care can be strengthened from a patient’s perspective. Once patient partners have committed to participating, they will receive the list of questions in advance of the interview. Please note that all responses will be kept strictly confidential and will only been shared between team members.
