As a congestive heart failure patient, we want to hear about your hospital discharge experience. Your valuable input through a personal interview will help us as we work towards improving the patient experience. Your assistance will help shape the future discharge and follow up process.

We are exploring how hospital discharge can be improved upon by bringing family physicians, and possibly other community specialists, into the process early on to help improve the patient experience. Through a personal interview over the phone or by video conference you will be asked questions related to your experience so we can improve upon processes that affect all heart failure patients discharged from hospital.Level of Engagement
This opportunity is at the level of consult on the spectrum of engagement. The promise to you is that the health care partner will listen to and acknowledge your ideas and concerns, and provide feedback on how your input affected the decision.Eligibility
• Open to patient partners from the Victoria area • Experience (personal or family) being discharged from the hospital after congestive heart failure • Comfort telling your story about hospital discharge experience • Please note:- Applicants must have previously attended a PVN orientation session and completed the Volunteer Agreement. If you have not attended an orientation session but you are interested in this opportunity, please contact Ashley Clark directly to see if accommodations may be possible.

• Number of vacancies: 4 • Date/Time: the interview will take place in August on a virtual platform (Zoom). The specific date/time is flexible based upon each patient partner’s schedule. It is estimated that an interview will take approximately 60-90 minutes • Location: Zoom • Commitment: One timeReimbursement

The Division of Family Practice is putting together a proposal to improve coordination of care for patients and their caregivers, including transitions between hospital and community-based care, and communication between family physicians and specialists involved in you care. This opportunity is to identify current gaps in care for patients with congestive heart failure who have been discharged from the hospital. Input from patient partners will be used to help us identify current gaps in care, and areas where the system of care can be strengthened from a patient’s perspective. Once patient partners have committed to participating, they will receive the list of questions in advance of the interview.
