As we prepare for transition to new St. Paul’s Hospital in 2027, we are developing Models of Care for each program. A Model of Care helps health care teams understand patient needs and how health care professionals can best work together to meet those needs.

We will soon be starting our work on the Emergency Department Model of Care and we are seeking 2 patient partners to help support the work. Patient partners work with the Professional Practice Team at St. Paul’s to provide their patient/family perspective to guide the development of a Model of Care.

The patient partner will be asked to review draft documents and provide feedback, as well as attend a series of meetings to discuss the Model of Care as it progresses.
