Project Aim: To reduce inappropriate and avoidable ED transfers, and to prevent unnecessary acute care admissions.

Patient population: Residents living in long-term care homes (LTC).

Commitment:
• An 8-month commitment, with approximately 2 hours of meetings and discussions per month
• Three 4-hour learning sessions (20-Oct-2025, 7-Jan-2026, 12-Mar-2026), in person at Vancouver, BC

Expectation:
• Provide input and recommendations on processes. If they can speak at learning session on lived experiences in the LTC Setting that would also be ideal but not required.
• Insights into the healthcare process from the patient’s, family, and caregiver perspective, including challenges, issues, risks, and suggestions on how to improve it. Needs from a family and caregiver perspective, resources, and ideas that can help improve quality of care in a LTC setting.

This opportunity is at the level of Involve on the IAP2 Spectrum of Public Participation.

Eligibility

  • Willing & comfortable to share health care experiences in a group setting
  • Specific health care experiences [details below]
  • Have access to technology and the internet to participate in the engagement opportunity
  • Comfortable using technology to attend online/virtual meetings
  • An interest in improving health care services
  • The time to participate in the engagement opportunity
  • Patient partners with lived experience at Long-term care home; can be family and caregiver.
Patient Partners ARE required to be PVN members to participate in this engagement opportunity.
