The Comox Valley Primary Care Network (PCN) is moving ahead with a new Governance model and a steering committee is being formed. We are seeking two patient partner strategic thinkers who can see the big picture and bring the patient voice in a collaborative fashion.

Aim

The vision for the integrated system of care is to create a coordinated delivery system for primary and community care that is person-centred, effective in meeting population and patient needs, and delivers a quality service experience for patients and their families – a system that is easy to understand for those who use it and those who work in it. PCNs are foundational to achieving this vision. Patient partners will be an integral part of the decision making process that will effect patient care in the Comox Valley. They wil work with the three partners - Division of Family Practice, Island Health and the First Nations Health Authority. They will attend the steering committee meetings, listen to and make recommendations, engage in discussion and be part of the decision making process.

Level of Engagement

This opportunity is at the level of collaborate on the spectrum of engagement. The promise to you is that the health care partner will work together with you to formulate solutions and incorporate your advice and recommendations into the decisions to the maximum extent possible.

Eligibility

If you have a strong interest in this work but have not yet completed a PVN orientation and volunteer agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact the engagement leader directly.

Logistics

  • Vacancies: 2
  • Meetings will usually be held at the Comox Valley Hospital (in-person).
  • Meetings will be approximately 2 times/month lasting 2-3 hours/meeting
  • Duration of project will be over a year
  • Specific dates and times to be determined with the first meeting date in late April or early May 2019

Reimbursement

Local travel (mileage/parking) expenses as applicable according to Island Health rates will be reimbursed. Further information will be shared with successful candidates.

Background

The Comox Valley primary care network is a new partnership between the Division of Family Practice, Island Health and First Nations Health Authority. As we start our implementation phase, we are morphing into a new governance model which includes a steering committee (decision making body), a sharing network (a large group of partner and community representatives who will make recommendations to the steering committee) and a variety of working groups. We are currently confirming the Terms Of Reference's for each group. This committee is the final decision making body and with 11 members total, plans to be nimble.
