Home health provides critical services to people in the community that focus on keeping people healthy and well at home. The coordination of those services with hospitals and local doctors is to make sure that they better respond to the needs of the population. Fraser Health is seeking two patient partners to lend their voice and experience to a redesign that is more patient- and family-centred.

Aim

By October 2018, Surrey/North Delta will implement a Home Health Redesign that aligns with the principles and attributes of the Primary Care Network initiative mandated by the Ministry of Health. Patient Partner will contribute their feedback, based on their lived experience as a patient and/or family member, focused on the impact of the design on patient and families to ensure it is patient centred, collaborative, and accessible.

Level of Engagement

This opportunity is at the level of consult on the spectrum of engagement. The promise to you is that the health care partner will listen to and acknowledge your ideas and concerns, and provide feedback on how your input affected the decision.  

Eligibility

If you have a strong interest in this work but have not yet completed a PVN orientation and volunteer agreement, are unsure if your experience is a good fit or feel another format of engagement would work better with your availability, please contact the engagement leader directly.

Logistics

Number of patient partners: 2 Date/time: Thursday Mornings, time to be determined Location: Fraser Health Corporate Office, Central City - Surrey Commitment: One week to six months, in person

Reimbursement

Pre-approved expenses related to travel to in-person meetings will be reimbursed according to the Fraser Health Travel Policy (including mileage, parking up to $10/day, public transit).

Background

The Ministry of Health has developed a Primary Health Care Charter. Fraser Health is required to ensure services align with this charter. Home Health services, therefore, are being redesigned to ensure they reflect this strategic plan. Committee members include the Director of Clinical Operations for Community Services in Surrey and a clinical and project lead.
