Member, Central Interior Rural Primary and Community Care Transformation Working Group

Interior Health and the Central Interior Divisions of Family Practice are actively seeking up to seven patient partners to participate in working groups from February until June 2019. The groups will bring recommendations forward to the Collaborative Services Committee. The working group will focus on Primary Care Provider attachment and access to Primary Care Providers, development of local partnerships, seamless transitions between services, and greater patient, family and caregiver involvement in their own care.

Aim

The goal of this initiative is to create solutions and transform health services in the Central Interior Rural region to enable easier access to health care services, create better linkages across the system of care, provide innovative health care provider retention strategies and deliver care that is person- and family-centred and is continuous, safe and coordinated. Patient partners will be asked to join working groups to help shape the Primary Care Networks and Specialized Community Services Programs in the Central Interior Rural Region. The working groups will evaluate and develop recommendations regarding the current health care services in the Williams Lake, 100 Mile House and surrounding communities including First Nations, Inuit and Metis communities within this geography.

This opportunity is at the level of collaborate on the spectrum of engagement. The promise to you is that the health care partner will work together with you to formulate solutions and incorporate your advice and recommendations into the decisions to the maximum extent possible.

Eligibility

• Experience in working within small groups with various disciplines in and outside of health care
• Comfort in telling stories and sharing perspectives
• Strong understanding and knowledge of Health Care Systems is a definite asset
• Access to email and phone and be responsive in a timely manner
• Able to join daytime or evening meetings in-person or via teleconference
• Connection to the rural communities of Williams Lake, 100 Mile House and Indigenous (First Nation, Inuit and Metis) communities in the surrounding area

Logistics

Vacancies: 7 Location, Date, Time: Meetings will be held every two weeks or monthly depending upon the level of involvement required. As the work gains momentum, the participation either in person or by email or phone may be more frequent. Meeting dates, times and location will be determined as this initiative gets underway. Commitment: Five months. This initiative is time-sensitive and must be completed by the beginning of June 2019. Further involvement may be requested past June 2019 to either continue to participate in working groups, steering committees or become part of the implementation of recommendations.

Reimbursement

Meals at the in-person meetings will be provided. Travel to and from meetings, hotel if required and meals if not provided as per government requirements listed in the expense claim form provided.

Background

This opportunity has come about due to the desire of Health Care Leaders, Physicians, partners and the Ministry of Health to achieve objectives of person-centered care systems for BC. Studies have shown how important it is for patients to have access to a Primary Care Provider in order to achieve the best possible health outcomes. Access to health services has been compromised over the past decade. To reverse the trends, governments are supporting the introduction of many innovative Primary Care initiatives aimed at improving access for patients. The team includes: Central Interior Divisions of Family Practice, Interior Health, patient partners, Practice Support Program, General Practice Service Committee, the Ministry of Health, First Nations Health Authority, local Indigenous and Metis band and community leaders, project managers (division and health authority) and other external partners.
