Improving Hospital Discharge – Rapid Process Improvement Workshop Team

Have you always wondered about the processes surrounding discharge from the hospital? We are looking for a patient partner to ‘rapidly improve’ processes that support safe patient flow through discharge.

Aim

Central to our core values is to keep the patient experience front and centre as we seek to better understand and improve processes. The patient partner will be a full member of the improvement team and relied upon to share and represent the patient experience of the current state and the impact of proposed improvements.

Level of Engagement

This opportunity is at the level of involve on the spectrum of engagement (www.iap2.org). The promise to you is that the health care partner will work with you to ensure that your ideas and concerns are reflected in recommendations and provide feedback on how your input affected the decision.

Eligibility

• You or someone you know has been a patient at a hospital; ideally having been admitted and discharged on a general medical unit
• Confident to seek and provide feedback
• Willingness to work on and off active hospital inpatient units
• Willingness and ability to work in a team and at times on your own
• Some understanding of quality improvement methodology e.g. PDSA cycles
• Volunteers must have previously attended a PVN orientation session.  If you have not attended an orientation but are interested, please contact the Engagement Leader directly to see if accommodations may be possible

Logistics

• 1 patient partner
• Primary Event is December 5-9 (M-Th 0800-1600, F 0800-1200) with Follow-up meetings at 30, 60, 90 days post RPIW
• In-person at Trail Kootenay Boundary Regional Hospital Education Room
• Information and education will be provided about the philosophy of ‘Lean’ and use provided tools to work in teams to make improvements over a one week period.

Reimbursement

Lunch costs are covered by Kootenay Boundary Regional Hospital.

Background

The opportunity involves participating in an activity (’Lean’ event) which includes observations of clinical processes; sharing opinions regarding improvements to processes; consulting with team and clinicians on the unit regarding improvements; implement improvements; participating in feedback and reporting out. The improvement RPIW team will include patients, administrators, nurses, allied health, doctors, observers and Lean experts.
