Fraser Health offers a wide range of home and community-based services, including urgent primary care centres, family doctors and home care.

Primary health care is the regular medical care you receive from your family doctor or nurse practitioner. It includes a broad spectrum of services and helps coordinate your ongoing care. The primary health care team at Fraser Health works closely with family physicians, specialists and other healthcare providers to ensure high-quality care. Other professionals in this team include public health staff, nurses, midwives, pharmacists, mental health professionals, physiotherapists, dietitians and home care workers.

Home Health clinics, available throughout Fraser Health, offer services like palliative care, wound care, IV medications, medication management, health education, and assessments for clients who can access services outside their home.

Patient and Family Advisory Councils offer patient partners the opportunity to meet regularly with Fraser Health leaders to promote and advance person-centred care and improve health care quality, safety, and experience.

The council meets for 1 hour each month, with summer and Dec breaks.

The Home Health and Primary Health Care Patient and Family Advisory Council has supported service delivery planning, patient flows, interior and landscape design. Recent activities include information and feedback about Smart hospital innovations (such as electronic medical records, real time location, and hospital at home). Also, the council has had the opportunity to meet in-person, at the construction site, to provide feedback on sample, mock-up, spaces for specific care areas (such as inpatient room, patient check-in, and exam rooms).

Share your experiences and ideas with leaders to make sure your perspective is considered in service planning, delivery, and evaluation. Fraser Health values meaningful dialogue with the people they serve to support the transformation of health care.

  • Willing & comfortable to share health care experiences in a group setting
  • Have access to technology and the internet to participate in the engagement opportunity
  • Comfortable using technology to attend online/virtual meetings
  • An interest in improving health care services
  • The time to participate in the engagement opportunity
Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
