St. Paul’s Hospital is looking for patient partners to help facilitate a virtual workshop about the transition to adult care. This workshop is designed for youth and young adults (ages 16-25 years) and their families to learn about what to expect in adult care and gets tips on managing their health care as adults. This workshop is also a chance for attendees to connect with peers and share experiences related to health care transition.
Patient partners will help facilitate these 1-hour virtual sessions alongside the Transition Program Leader (Emma Iacoe). There will be 4 workshops offered in 2025, and the content is already developed. Your role will be to deliver the workshop content and share your personal experiences during discussion.
We are looking for youth/young adults (ages 18 – 30 years) who have lived experience with a health condition and have experience with health services. Ideally, we would like youth/young adults who are currently transitioning or have recently transitioned to an adult care program (at St. Paul’s Hospital or elsewhere). An honorarium will be provided as a gesture of gratitude.
