We are working to create educational material focused on optimizing patients’ discharge from hospital with respect to their medications. In some of our previous research we have identified that patients often find the time of discharge from hospital to be confusing, especially in relation to medication changes that may have occurred during their hospital stay.

We have created some paper based educational material and a short, patient informational video and are seeking to get patient input. The material would be provided to the patients electronically via email and they would be asked a few questions pertaining to the material in addition to whether they have any other feedback that they would like to provide.

Eligibility

  • Specific health care experiences [details below]
  • Have access to technology and the internet to participate in the engagement opportunity
  • Comfortable using technology to attend online/virtual meetings
  • An interest in improving health care services
  • The time to participate in the engagement opportunity
  • Would be beneficial if patients had experienced a hospital stay/discharge so they could reflect on their experiences in relation to the created material.
Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
