The Chilliwack Division of Family Practice, Fraser Health, Indigenous Leaders, Municipal leaders, and other local partners aim to improve the health of the community. These organizations acknowledge the benefits of regularly connecting and collaborating to work towards meeting the needs of the community.

Collaborative Services Committee (CSC): A regular meeting with community partners to:
• Integrate and collaborate with community health care, hospitals, and specialized programs and services (such as those for mental health and substance use and complex, medically frail).
• coordinate and plan how Fraser Health can best support care in the community.
• discuss how the community can come together to support challenges in hospitals and other Fraser Health care services.

Meetings are quarterly (once every 3 months – for a total of four meetings a year) from 5:00- 7:00. Most meetings are at Chilliwack General Hospital and some may be virtual using MS Teams

Objectives for this opportunity are:
• Advance person- centered approach to care
• Improve the patient experience
• Improve quality and patient safety

• Chilliwack Division of Family Practice representative
• Fraser Health Chilliwack health services senior leader
• Municipal Government representative
• Local Indigenous partners
• Other local partners

This opportunity is at the level of Consult on the IAP2 Spectrum of Public Participation.

  • Willing & comfortable to share health care experiences in a group setting
  • An interest in improving health care services
  • The time to participate in the engagement opportunity

Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
