Background
The Burnaby Primary Care Networks (PCN) is a part of the BC Ministry of Health’s Primary Care Networks initiative. Burnaby’s PCNs are unique in that they extend well beyond the proposed scope of health care services to include the full spectrum of integrated health and wellness in Burnaby and to address barriers to the social determinants of health across the City.
As such, the Burnaby PCN is a true tripartite partnership made up of the Burnaby community partners represented by the Burnaby Interagency Council (BIAC), the Health Authority (Fraser Health) and the family doctors represented by the Burnaby Division of Family Practice (BDFP). This partnership is reflected in Burnaby PCN’s governance and decision-making processes, as well as the partners’ active participation in: Burnaby PCN Local Leadership table!
