Patients that have recently been discharged from hospitals are at high risk of medication errors, adverse medication events and re-admissions. Pharmacists are well-positioned to support patients undergoing transitions of care through medication reconciliation and assessment, bridging communication gaps and providing patient education.
The intent of this project is to build processes to support proactive identification of recently discharged patients and building a referral pathway for these patients to be seen proactively by a primary care pharmacist within the Chilliwack and Fraser Health Rural Primary Care Network. This practice change will reduce administrative burdens, improve integration of primary care pharmacists within family practice teams, and support timely and patient-centred care.
We are seeking an additional patient partner to collaborate with us on this project.
Meetings will be scheduled every 1-3 months and will be held virtually by Microsoft Teams, with an option to call in by phone. Meetings will be scheduled on dates and times most convenient for all team members, and will be no more than 1 hour in duration.
Since 2021, BC has incorporated primary care pharmacists in Primary Care Networks (PCNs), including in the Chilliwack and Fraser Health Rural Region.
