Evidence shows that inappropriate antipsychotic prescribing leads to increased risk of adverse effects such as falls and strokes, longer acute stays and increased readmissions between LTC and acute. Work is underway at VCH & PHC to address prescribing practices for seniors in both LTC and acute setting starting with Lions Gate Hospital as the initial pilot site of interest. There is a gap in appropriately prescribing (especially deprescribing) antipsychotics for seniors transitioning between acute and LTC. There is opportunity to explore this gap and quantify its scope, contributing factors and downstream effect for acute care and LTC patients.

Shared Care has recently endorsed this project and had provided funding for the exploration phase of work. The project team is seek for a patient partner who may have experience in transitions of care, LTC or senior care and ideally would be familiar with the Northshore area to bring their perspective and lived experience to the project. The time commitment behind this project is approximately 1hr/month between February and August 2026.

Interview/ Discussion will be held between Feb 16 to Mar 16, 2026.

  • The time to participate in the engagement opportunity
  • Patient partners will contribute lived experience and perspectives related to senior care, long term care, and transitions between care settings.
Patient Partners ARE NOT required to be PVN members to participate in this engagement opportunity.
