ROI form is used for patients to request their own medical records and we have done recent significant improvements to the form (public facing) with the goal of making it user friendly and in plain language.

We are hoping we can have a couple patients walk through the form and provide feedback.

This opportunity is at the level of Collaborate on the IAP2 Spectrum of Public Participation.
