Northern Health is standardizing the materials patients receive when they are admitted to the hospital. Patient partners are invited to join the committee and share what they feel patients need while transitioning in/out of the hospital.

Aim

To engage patient partners in creating materials provided to patients and their families when admitted to hospital.

Level of Engagement

This opportunity is at the level of collaborate on the spectrum of engagement. The promise to you is that the health care partner will work together with you to formulate solutions and incorporate your advice and recommendations into the decisions to the maximum extent possible.

Eligibility

  • Experience being in the hospital or supporting a family member in the hospital.
  • Experience using a computer and able to view PDF documents.

Logistics

Vacancies: 2-4 Location, date, time and frequency:
  • Dates and times of meetings will be set based on committee members availability.
  • Meetings will be held during the hours of 8 am-4 pm.
  • Meetings will occur once every two-four weeks until the material is developed, and will be held by phone

Reimbursement

There are no anticipated expenses.

Background

NH wants patients to be as informed as possible about what to expect once they arrive in the hospital, and what to expect when they are discharged. They want to set people up for success so that once they are discharged from the hospital, they have the supports in place and the knowledge they need to continue on their health care journey. Other staff members from Northern Health leadership will be involved in the review and approval process of the documents. This care setting is directly related to acute care.
