On behalf of the PHSA Ethics Service, I invite you to review the attached draft PHSA Patient and Family Decision Support Tool. This tool is intended to provide patients and families with some additional structure and guidance as they work through challenging health care decisions for themselves or a loved one.  The tool may be used either by the patient/family on their own or with support (for example, from a trusted health care provider or a member of the PHSA Ethics Services team).

We invite you to provide feedback on the overall structure and the questions posed in each section. Your input and suggestions are extremely valuable and will help ensure this tool is useful for patients and families across PHSA.
