The North Shore Palliative Care Program would like to improve transitions in care for palliative clients, including enhancing communication between the teams and the patient/families/caregivers, as well as increasing patient collaboration through goals of care conversations.

The Project Team would like to recruit 2 CEAN members who have personally been involved with transitions of care either as a family member/caregiver or a client/patient (eg. hospital to community, or hospital to long term care towards end of life).

If you are interested in being a part of this work to support improved transitions in care for patients and families/caregivers, Please click here to fill out an application by end of day Friday August 14th, 2020.

Please contact ce@vch.ca if you have any questions. Thank you!
