The North Shore Palliative Care Program would like to improve transitions in care for palliative clients, including enhancing communication between the teams and the patient/families/caregivers, as well as increasing patient collaboration through goals of care conversations.

The Project Team would like to recruit 2 CEAN members who have personally been involved with transitions of care either as a family member/caregiver or a client/patient (eg. hospital to community, or hospital to long term care towards end of life).

Several extremely engaged PVN members expressed interest in being part of piloting a patient virtual care peer support system. I am confident that this is the beginning of many more exciting collaborations!
