Help Identify Priorities for Chronic Disease Care in BC

The Shared Care Committee (a partnership of Doctors of BC and the BC government) is asking health care providers and those involved in caring for individuals with chronic diseases to complete a short survey (4-6 mins).

Feedback will provide valuable information to inform the strategic priorities of an emerging Chronic Disease Community of Practice (CoP).

The CoP will bring together family physicians, specialists, and other partners to share resources, and to work collaboratively to guide, coordinate and advance care around key areas of chronic disease care in BC.

Whether you are a health care provider, health authority representative, community organization leader, or an organization representing patients, families and caregivers, your input will be key to this unique collaboration.
