Member, Urgent Primary Care Centre Working Group

The Urgent Primary Care Centre Working Group seeks to shape what services the centre will offer to ensure community members can access good quality health care in their community, across all ages and needs.  Do you have experience with health care that might help shape the urgent primary health care in Surrey? Aim

Patients partners will provide input in planning and implementation so that the outcomes lead to improved patient care in Surrey. They will participate as a decision maker in how the Urgent Care Centre services are designed and delivered. The experience of patient partners will inform services so that they are relevant, accessible, meaningful and centred around patients, families and caregivers.

This opportunity is at the level of collaborate on the spectrum of engagement. The promise to you is that the research partner will work together with you to formulate solutions and incorporate your advice and recommendations into the decisions to the maximum extent possible.

Eligibility

• Ideally, have knowledge and experience with community health care services in North Surrey
• Are comfortable with public speaking in a group setting
• Have sat in a committee previously (desired but not mandatory)

Logistics

Vacancies: 1-2 Date/Time: Meetings are during the day, in North Surrey, for 2 hrs, and occur approximately every three weeks. Location: North Surrey Commitment:  A combination of in-person meetings, teleconferences and webinars over 6 months. Agendas and meeting notes are circulated and will require extra time to review. Additional correspondence via e-mail in between meetings may also be required.

Reimbursement

Preapproved expenses, per Fraser Health Travel Policy, related to local travel to in-person meetings will be reimbursed including mileage, parking, tolls and/or public transit.

Background

The Surrey Urgent Primary Care Centre will have a team of doctors, nurses, nurse practitioners, pharmacists and other health-care professionals working together to the full extent of their skills to provide a broad range of services. At full staffing, this centre will be able to accommodate up to 1,300 unique patient visits per week, and attach up to 5,000 patients over time. Among these patients will be vulnerable residents who have complex care needs, including the frail and elderly, people needing specialized mental health and substance use services, and the North Surrey/Whalley community. The centre will also provide diagnosis and care for non-emergency conditions requiring medical attention within 12-24 hours, including lacerations, earaches, back pain, and sore throats. When the centre opens, area residents can visit the location and have their needs assessed by a nurse or other team member. People may also be directed to the centre by community- or hospital-based care providers. A variety of appointment options will be available, including one-on-one visits, appointments with multiple providers, telehealth visits, home visits, and group appointments to support people with similar conditions. The centre will also provide outreach services, by connecting nurses to community locations such as shelters. The hours of the Surrey Urgent Primary Care Centre will be daily from 10 a.m. to 10 p.m. The centre will be located at 9639 - 137A Street, near specialists’ offices, supportive health-care services and Surrey Memorial Hospital. The proximity to the hospital will make it easy for hospital staff to refer patients to the centre for follow-up care, when appropriate. As well, the centre’s staff will be able to easily direct people to hospital-based services, when necessary.
