Do you have an interest in improving the acute care experience for patients and families? Kelowna General Hospital is looking at making improvements in hospital care and discharge planning.

Aim

The aim of this opportunity is to improve the care and discharge planning process through an improvement workshop.

Level of Engagement

This opportunity is at the level of collaborate (at a local community or program level) on the spectrum of engagement (www.iap2.org).  The promise to you is that the health care partner will look to you for advice in decision making, developing alternatives, and solutions and include your recommendations into the decision as much as possible.

Eligibility

  • Experience as a patient in acute care or supporting a family member
  • Comfortable sharing experiences
  • Patients should have previously attended a PVN orientation session.  If you have not attended an orientation but are interested please contact the Engagement Leader directly

Logistics

  • Vacancies: 1-2
  • April 3-7, 2017 (8:00-4:00)
  • Meetings are held in-person at Kelowna General Hospital
  • The patient partner will be invited to participate in the 30,60 and 90 day follow up sessions
  • If you are unable to attend all the workshop days, please indicate your availability

Background

Acute care teams are constantly looking to improve the patient experience by improving process and identifying opportunities to partner with patients. The process of care planning and discharge planning is integral to a positive experience for patients and family through the health care system. The health system relies on quality efficient, effective processes - this workshop will work to improve in those areas. During the five days, you will learn about Lean (see definition below), and work with KGH staff to understand the current state, identify ideas to improve the process flow and eliminate waste within the process while adding value to the patient experience. Participants in this workshop may include:
  • Nursing (RN or LPN)
  • Unit Clerk
  • Patient Care Coordinator
  • Nurse Manager
  • Physiotherapist/Occupational Therapist
  • Social Work
  • Transition Liaison
  • LEAN Consultants
  • Practice Lead, Regional Initiatives
  • GP(s) – family physicians
  • Interior Health feels the patient voice is integral to their improvements to ensure they are not just meeting the organization’s needs, but the main stakeholder as well.
 Definition:
  • Lean: Lean is a systematic approach to identifying and eliminating waste or non-value added activities through continuous improvement. The focus of this work is through the eye of the patient. Patient perspective is key to ensure the improvements made truly eliminate waste and add value to the process.
