The Virtual Care Strategy branch of the Ministry of Health is developing a provincial virtual home care strategy and we need the patient voice! We are looking to engage and consult patient partners through review of three online surveys to ensure our survey centres the patient.

Lead Organization/Department

Ministry of Health, Virtual Care Strategy Branch

Aim

Our Home Health Monitoring (HHM) projects engage clients and their families to support patients’ quality of life by educating patients and caregivers about chronic conditions and symptoms, and giving patients the freedom to self-manage their condition(s) from their homes and away from traditional hospital settings.

We recently created five surveys: three for patients with chronic disease (e.g. heart disease, COPD, multi-morbidity), one for caregivers and one for care providers to assess their experience and satisfaction with the HHM program during and after completion.

We would like to involve patient partners to review our surveys and give us their feedback on the appropriateness of the survey questions.
