Background

Local community services have changed with Northern Health moving to Team Based Care approach. Services are becoming more integrated, working together to support patients needs. This process is currently in a pilot program with Northern Health and local physicians. The Integration Leads and the Community Services manager would like to have the perspective of patients in navigating the options for services.

I have learned so much about innovation and capacity building by being part of PVN. I signed up to build my leadership skills, however, I experience benefits for myself, my community and Indigenous peoples throughout the province. I understand more about the complexities and opportunities required to improve health care.
