Aim

There is often a significant delay between when a client vacates a bed to the time the next client occupies the bed. This can translate into increased acute stays, increased risk to community clients and increased stress for clients and their caregivers. By making the processes more efficient, we can improve access and flow from acute into long-term care and reduce the wait time for a long-term care bed. This will increase the instances where clients are receiving the right care at the right time.

Background

Home Health is trying to bring efficiencies to the process, reducing the time the bed is vacant and reducing steps in the process. This improvement event will stream line the process for community, acute and long term care, improving access and flow from acute and community into long term care homes. This improvement workshop will focus on the period of time when a bed is vacated by a resident until the time the bed is occupied by the next resident. We have been experiencing an inefficient process for clients to access long term care. The process is complicated – it touches community, acute, long term care, plus a variety of human factors; the goal is to bring a team together that is dedicated to finding solutions in a collaborative manner.

Recently, a PVN Patient Advisor asked healthcare partners, “What are we learning from patients, families, and others thus far during this pandemic?”. This is an excellent question and one to ask ourselves daily.
